You are asked for consent at the beginning of the evaluation for Inclusive Intakes to process your intake.   There is again a step at the end of the assessment where you are reminded you can abandon the assessment if you changed your mind about submitting.  

  The clinic will put your intake into your medical record. Inclusive Intakes does not save your intake or any of your information.  By design, we don't have the ability to retrieve previous assessments. If you wanted to see your intake, you would need to request it through the clinic's medical records department.

These are voluntary intakes, which you don't have to complete. Your provider can ask you these questions during your visit. 

Completing might just get you quicker from assessment to treatment planning.
